The starting note

Begin with your concern.

You can ask for help without knowing which treatment, if any, will fit.

Name what you hope will change

Write down the problem that sent you looking. Is it a symptom, a weight concern, or a claim you saw online? Use everyday words. Note any steps you have tried so far, along with your questions.

Bring a list of medicines and supplements to the conversation. Include questions you feel awkward asking; they still belong in the visit.

Find out what the visit includes

You receive care remotely with telehealth, through video calls, phone conversations, or messages online.

The Federation of State Medical Boards addressed medicine choices from a list in May 2026 guidance. Choosing from a list without a proper health assessment falls short of proper care.

This means picking a treatment as if from a shopping list. It does not mean answering questions about your health.

Find out who reads your health history and how to put questions to them. If the online format feels hard to use, an in-person clinic is another way to start the conversation.

Get the names behind the offer

Ask for the name and job of the doctor, nurse practitioner, or other health professional who would assess you. Write down that name, the exact medicine, and the pharmacy that would supply it. Keep the answers together to compare offers.

Get a complete cost breakdown

Request a written breakdown of the visit, ongoing care, medicine, tests, and delivery. Find out what you must pay when an assessment results in no prescription.

A question to keep

What happens after the assessment if medicine is not offered?
